This form may be used by any member of the Mitchell Hamline School of Law community to report an incident of sex discrimination or sexual harassment including sexual assault, dating violence, domestic violence, or stalking. We encourage you to:
- Reach out to people and services listed in Section A. for assistance and support; and
- Complete the Sex Discrimination and Sexual Harassment Reporting Form in Section B.

Section B: Sex Discrimination and Sexual Harassment Reporting Form
The questions included in this form are optional and will be used for statistical monitoring in compliance with state and federal reporting requirements. This information will also assist Mitchell Hamline School of Law in addressing and preventing incidents of sexual misconduct in the law school community.
All information reported on this form will be forwarded to the Title IX Coordinator, but does not mean that the law school will initiate an investigation into the complaint. In order to investigate a complaint, there must be sufficient information to pursue. If the identity of the person who experienced sexual misconduct is reported, the Title IX Coordinator or the Assistant Title IX Coordinator will contact this individual to offer support services, interim measures, and will discuss the option of initiating a formal complaint under the law school’s policies.
