What to Bring – Modifying Custody and Parenting Time
Please bring the following items to your appointment. The items in bold are essential; all other items you should bring if applicable to your situation.
- Current Custody and Parenting Time Order
- Documents relating to Order for Protections, Domestic Abuse, or Child In Need of Protection or Services (CHIPS) proceedings
- Demographic details for you and other parent including: Name, Birthdate, Social Security Number, Address, Phone Number
- Verification of public assistance for the child(ren)
- Proof of income
- Health care coverage information for the child(ren)
- Verification of child support for the child (if previously ordered)
