Procedures for requesting electronic textbooks or course materials

Students who intend on requesting Alternative format textbooks must complete the form below and provide complete textbook information along with copies of your book receipts. If you have any questions, please contact Disability Services. If there is an issue, Disability Services will contact the student.

Book Request

Please input the course code. This will auto-populate the Course Title and Instructor fields. Please review the Course Title and Instructor fields for accuracy to ensure the correct course code was input. Full course codes may be found in the title of the course on Canvas. Alternatively, you can find the information on the Agresso Course Information Page. When using the information from Agresso, course codes are formatted from Crse#_TERMSect#.
